Your details
Your full name, date of birth, residential address and contact details, and a short summary of your matter.
Three forms of identification
Clear photos or scans of three identity documents, for example a passport, driver licence and Medicare card. Photos taken on your phone are fine, as long as every detail can be read.

If Vamp Law has sent you forms to fill in, you can upload the completed copies here too.
This page is for individual clients. If you are instructing Vamp Law on behalf of a company or another organisation, contact us and we will tell you what is needed.
